Once you've made your first Backup to an Ext Drive. A week later I want to back up the computer drive to reflect any changes. Does ViceVera make an entirely new Backup erasing the first backup?

No, only changed files (based on their modified timestamp) will be copied as an incremental operation. There are a lot of options to customize program behavior and execution methods.

Would it copy the changes only with all the permissions ?

Yes, if NTFS permissions are set to be copied, if a file only changes permissions, then only permissions are copied (not the whole file again).
